'What is the pancreas?'

It is a solid organ positioned behind the stomach in the upper part of the abdomen.

'What are the most common problems that affect the pancreas?'

Pancreatitis is the most common pancreatic condition, categorized as either acute

'What causes these pancreatic conditions?'

Acute pancreatitis is caused by alcohol intake, gall stone, hyperlipidemia, hypercalcemia, drug-induced, auto-immune or in some cases cause is unknown. Chronic pancreatitis can be caused excessive use of alcohol. Also, a condition arising at birth that leads to a disconnect in the pancreatic duct system, called pancreas divisum, makes some people susceptible to chronic pancreatitis in their adult years. Cystic fibrosis, a very common hereditary disease contracted at birth, may also cause chronic pancreatitis. Pancreatic cancer occurs without any apparent reason, though patients with chronic pancreatitis are more likely to contract it. Pancreatic cancer is also quite common among patients with the hereditary form of pancreatitis.

'What are the warning signs of pancreatic problems?'

Pancreatic conditions can produce severe pain. The pain is usually in the upper middle part of the abdomen, but it can also be in the upper left side, upper right side or throughout the abdomen. Pain often travels straight to the mid-back region. Even though most diabetes is unrelated to pancreatic conditions, if it occurs at the onset of upper abdominal pain, then pancreatic problems are the likely source. In addition, if the patient experiences loss of weight, either pancreatitis or cancer should be considered.

'If I suspect a pancreatic problem, what should I do?'

If the symptoms are severe, seek immediate medical help. At the very least, a series of tests should be performed to establish a diagnosis. A blood test that shows the presence of pancreatic enzymes will suggest acute pancreatitis, but not necessarily chronic pancreatitis. In that case,EUS is the ideal modality today. Recently EUSis more-specific test that is available and is minimally invasive and can clearly show pancreatic swelling and inflammation. EUS guded FNA has the advantage of providing definitive tissue diagnosis for specific pancreatic conditions. EUS-ERCPinterfaceis an unique way of providing agnosis, treatment or palliation for various pancreas related diseases.

'What are the symptoms of Pancreatic Cancer?'

In the early stages, pancreatic cancer is extremely difficult to detect because often there are no symptoms. But, as the cancer grows, symptoms may include:Pain in the upper abdomen or upper back, Yellow skin and eyes, and dark urine from jaundice, Weakness, Loss of appetite, Nausea and vomiting, Weight loss These symptoms are not sure signs of pancreatic cancer. An infection or other problem could also cause these symptoms. Only a doctor can diagnose the cause of a person`s symptoms. Anyone with these symptoms should see a doctor so that the doctor can treat any problem as early as possible.

'How is Pancreatic Cancer Diagnosed?'

The most accurate modality of diagnosis of early pancreatic cancer in Endoscopic ultrasound. Endoscopic ultrasound has the advantage of picking up pancreatic tumors at early stages when curative resection of the tumor is possible. So in high risk patients with family history of pancreatic cancer and history of chronic pancreatitis should undergo screening EUS in regular interval. EUS has the advantage of performing EUS-FNA of suspected pancreatic mass for tissue diagnosis.

Other modalities of imaging that help in diagnosing the pancreatic cancer are CT scan, MRI and some blood investigations for tumor markers.

'What are the available Treatment Options?'

People with pancreatic cancer may have several treatment options. Depending on the type and stage, pancreatic cancer may be treated with surgery, radiation therapy, or chemotherapy. Some patients have a combination of therapies.

'What is Palliative Thearpy?'

Palliative therapy aims to improve quality of life by controlling pain and other problems caused by pancreatic cancer. To relieve the jaundice ERCP is performed. And with deployment of self expandable metallic stent in the bile duct, patient is relieved of the obstructive jaundice effectively and on a long term basis. Duodenal obstruction due to pancreatic cancer has also been palliated with endoscopically by placement of self expandablepyloro-duodenal metallic stent.

'How is Pain Controlled?'

The use of opioids (or narcotics, the strongest pain relievers available) is the main way to treat pain from pancreatic cancer. Other types of medicines used to relieve pain that are not opioids are: acetaminophen and non-steroidal antiinflammatory drugs (NSAIDs).

Nerve block

The doctor may inject alcohol into the area around certain nerves in the abdomen to block the feeling of pain with help of endoscopic ultra sound.


High-energy rays can help relieve pain by shrinking the tumor.
